What it already knows

Good questions

How do I get my records in?
Fastest: photograph your insurance card and driver's license. Most complete: download a health summary from your patient portal (MyChart and every Epic hospital, Cerner, athenahealth, Kaiser, the VA — look for "Download health summary"). Or photograph a form you already filled out by hand — Formova learns the answers.
Where is my data stored?
Encrypted, in your Formova account, so it's there on any device you sign in to. Your sources — card photos, exports, form photos — are kept so you can check where anything came from, and you can delete any of them. Nothing is sold, shared, or used to train anything. Delete your account and every byte goes with it.
What if the form has handwriting or a weird layout?
Formova reads the printed structure — labels, blanks, checkboxes, tables — from a photo. Very faint copies or heavy handwriting can confuse it; retake the photo in good light and straight on.
What about things my record doesn't have?
Those blanks are highlighted in yellow. Formova suggests an answer when it can; otherwise you type it once and it's remembered for every form after.
Can I just hand over the printout?
Yes. The printed form matches the original's layout with your answers typed in. Sign it, date it, done. Most front desks prefer it to handwriting.
